Holy Family Hospice

Purpose: To provide hospice services to those who are terminally ill and choose to stay at home or in an extended care facility. Holy Family Hospice offers an interdisciplinary team approach that helps patients and families cope with the complexity of the end-of-life process.
Population served: Residents of Cuyahoga County who are terminally ill.
Criteria: Must have certification from a physician stating that patient is terminally ill and, given the normal course of the treatment process, the patient has a life expectancy of six months or less.
Fees: There is no charge for this nursing home.
Major Funders: Accepts Medicare, Medicaid, private insurance and donations./p>
